Tweak comments in hook-templates.

* libsvn_repos/repos.c (create_hooks): make hook template comments
explain that 'start-commit' and 'pre-commit' return STDERR to client
upon failure.

* doc/book/TODO: ask cmpilato to document this in chapter 5 at some point.

Consistify the format of our book id="" attributes.
