Version Control With Subversion Source

Remove from TODO a fix that has already been made.

* doc/book/TODO: rewrite book's TODO based on last 2 months of user comments.

* doc/book/book/foreword.xml: Happily shill for the Mother Ship.

CollabNet effectively subsidized a lot of the time Ben, Mike, and Fitz
spent gathering usage information, not to mention all the time spent
actually developing Subversion of course. Therefore mention CollabNet
in the foreword, after checking that the authors are okay with this.

Fix a documentation typo reported by Joey Smith <joey@...>.
