Add Martin Further, Robert P. J. Day, and Steffen Prohaska to the
contributors list.

Lose the <firstterm> tags around meritocracy.

* doc/book/book/ch04.xml
Fix uses of short subcommand names. Also, add log message to
"Resurrecting Deleted Items" example. Patches by Francois
