Merge from ^/trunk/en r5572, r5573, and r5574 (hook script markup tweaks).

Followup to r5572 and r5573 with still more de-marking-up of hook script names.

Followup to r5572 with another de-marking-up of a hook script name.

Remove <filename> and <command> wrappers for hook script names
(pre-commit, post-unlock, etc.), and refer to them without additional
markup except where referred to as a literal filename or configuration
file section title.
