Version Control With Subversion Source

Continue renames begun in r8735:

* doc/book/book/appb.xml: New file, renamed from...
* doc/book/book/appc.xml: ...here, which is now temporarily removed.

Step 1 of N in removing most of cvs2svn material from the book.

This is part of a general shifting of the appendices. We're removing
most of the content about cvs2svn and replacing it with pointers to
more up-to-date information, since cvs2svn is still changing. The
pointers don't deserve their own appendix, so the current Appendix B,
which is dedicated to cvs2svn, will just go away, and the new
information (including mentions of the other converter tools) will go
in "Third Party Tools", with a ref from "Subversion For CVS Users".

Due to issue #1516, I can't do all these renames at once, hence there
will be several commits.

* doc/book/book/appb.xml: Removed.

* doc/book/book/ch05.xml: Anticipate new link to cvs2svn information.
The new link is bogus until this change is finished, of course.

Fix typo noticed by Chad Whitacre (chad@...).

* doc/book/book/ch05.xml: Fix typo

More book cleanup.

* ch06.xml (basic apache configuration): add warning about common 301
error scenario.

* ch07.xml (config): document [tunnels] section by adding xref to ssh
section of chapter 6.

* TODO: make to-do's to-done.

Filling up more holes in the book's coverage.

* ch04.xml (Repository Layout): don't even mention the idea of laying
out a repository indexed by branch. Make a xref to cmpilato's stuff
about 'project roots'.

(Best Practices): split into 2 sections about manual merge-tracking
and using --dry-run to preview merges.

(Working with your branch): make a quick forward xref to 'svn
switch' section.

* ch07.xml (servers): document all the ssl- variables in 'servers' file.

(miscellany): add missing varlistentry tag to autoprops
section. Document "log-encoding' and 'use-commit-times' variables.

Rip through some book TODOs in anticipation of 1.0.

* doc/book/TODO: Update.

* doc/book/book/appa.xml: Warn users about checking out the *whole*
repository and discuss how Subversion does auth.

* doc/book/book/ch07.xml: Document auto-props.

* doc/book/book/ch09.xml: Double check (and fix) switches for svn
subcommands and document svn blame, --ignore-ancestry and
--notice-ancestry.

Book tweaks.

* ch03.xml, ch04.xml, ch05.xml, TODO: mini-tweaks and corrections.

Remove some "handled" stuff.

Fix lots of little grammar and spelling and wordage issues.
