lose the bit that shows us creating one. While here, fix some
<indexterm> placement. (This takes care of the concern noted in
issue 66 ... or rather moves the concern elsewhere.)

Fix issue 43 by using a simple hyphen (-) instead of the −
character entity. While here, mark some text as <replaceable>.

Thicken up with prose the boring nested list of basic work cycle
elements.

Correct some title capitalization.

Merge and expand some similar sections around authentication,
password caches, etc.

(Help!): Rework this section a bit.

Minor tweaks.

Major reworking, mostly of chapter 1, but elsewhere, too. This is
my first pass at implementing fixes for a slew of organizational
grievances I've long held about this text.

Update pre-processing needs.

Remove some <firstterm>'s which now appears in Chapter 2.
