* doc/book/book/appa.xml:
Updated the section about the status codes to show the most common ones.
Added reference to other sections for more details.
Left in a comment about the CVS update codes discussions that should
be moved elsewhere.

Sometimes, some of the PNGs in doc/book/book/images/ shows up as
black squares in stead of pictures.

* doc/book/book/images/ch04dia1.png: Adjusting the file to look as a
image again in svn-doc.chm (the Windows HTML-help file)

Modifications to the book.

changes that are still relevant from his two messages linked from issue 1187,
except for the change to the list of status codes in appa.xml.

Corrected some more grammatical mistakes.
Corrected a renamed option on svn status in the appendix.
Split long <screen> lines into multiple lines using Unix continuation char.
