Second round of copyedits for chapter 2.

I didn't reflow any of the paragraphs that I edited, and I have to
say--it almost killed me. M-q is as much of a reflex for me as
blinking my eye when someone is trying to poke their finger in it.

Enter second half of 2nd-round copyedits (most of them) to chapter 4.

* ch00-preface.xml: remove periods from non-sentences in varlistentries.

Enter first half of 2nd-round copyedits (most of them) to chapter 4.

Enter 2nd-round copyedits (most of them) for Appendixes A and B.

Enter 2nd-round copyedits (most of them) for Chapter 1.

Enter 2nd-round copyedits (most of them) for Foreword and Preface.
