Version Control With Subversion Source

Add --reintegrate option to ch09 reference and switch old and busted
example to use it (new hotness).

This fixes issue #96, reported by khmarbaise

* ch04-branching-and-merging.xml: bring 'log -g' examples up to date with 1.5-rc5.

* ch06-server-configuration.xml: fix issue 103.
