Version Control With Subversion Source

Begin making copyedit changes suggested by O'Reilly. Love the
results, but the process is putting a crick in my neck!

Add notes about how to properly transform our Docbook XML.

* ch04-branching-and-merging.xml: enter O'Reilly's copyedits.

ch01-fundamental-concepts.xml: enter ORA's copyedits.

Phrasing tweaks and typo fixes.

Sync the Norwegian and English page.

* www/index.nb.html: Merged r2869, r2870.

* www/old-versions.nb.html: Translated to Norwegian + set svn:externals
to Id + added Id, base href element, and Vim magic.

Merge r2849 into the Norwegian page — move the old book listings into
their own file.

Validation fix: add xmlns attribute to the html element to make
