Expand some wording about revisions within branches, from Jack Repenning.

Fix trac issue 31.

* src/en/book/ch06-server-configuration.xml
(svn.serverconfig.pathbasdeauthz): add sidebar explaining oddness
with HTTP not re-authenticating partway through a checkout.

Updates to Appendix C, for the svn 1.4 book.

* src/en/book/appab-svn-for-cvs-users.html
- Minor rewordings, mostly from offby1's review.

* src/en/book/copyright.xml: change (c) date to cover 2007.

Updates to Appendix B, for the svn 1.4 book.

* src/en/book/appab-svn-for-cvs-users.html
- Minor rewordings.
- Link to the reference section on 'svn status' codes.
- Don't offer many repository converter tools; take a stand and
just recommend cvs2svn. Duh.

Update the 'quickstart' appendix for svn 1.4 book.

* src/en/book/appa-quickstart.xml
- Reword some things.
- Take some suggestions from offby1's review.
- Point out that one may need both the svn tarball and the
svn-deps tarball to build.

Clarify explanation of top-down vs. bottom-up learning.

* src/en/book/ch00-preface.xml
(svn.preface.howread): Add paragraph about top-down vs. bottom-up
learning. I'm tired of talking about this subject with in every
conversation I have with people about our book. I think it's useful
meta-philosophy.. :-)

* src/en/book/appa-quickstart.xml
(svn.intro): Remove terminology from here, now that it's mentioned
in the Preface. Simplify the appendix introduction, wrap the 2nd
paragraph in a <warning>, so impatient people actually see it.

Remove no-longer-necessary TODO file. We'll use Trac from now on.

* it/book/ch07.xml
Fix a slew of entity syntax errors and tag omissions. I can build the
IT translation as PDF now.

Fix all instances of '&mdash ' to be the well-formed '—'.
