Oops. Missed 'djh' and 'nsd' in my last username purge.

Remove from COMMITTERS (and the auth system) English users who got
grandfathered commit access by virtue of having full commit access on
the Subversion repository at the time the book was split off from it,
but who've never committed to the book sources:
