* ch02.xml: add a sidebar explaining how repositories are accessed
via URLs. This is probably the best place to introduce this, since
1) it's a basic concept, and 2) it's the first time we see a
repository URL in the book.

* appd.xml: fix typo.

It would be Really Swell if jerenkrantz could quickly look over this
chapter. I'd like to make sure I didn't mess up my explanation of

Whoops. Need to 'rm -rf' the html-chunk directory. Thanks to Jani
Averbach (jaa@...) for pointing this out.

* /doc/book/Makefile: (book-clean) force remove the html-chunk
directory.

* appd.xml: new appendix, all about WebDAV/DeltaV and Autoversioning.
Not yet finished, but will be soon. Need to pick jerenkrantz' brain
about the httpd-2.1 generic-locking he created. Will finish this
later tonight.

* ch07.xml (WebDAV): hack this section down quite a bit, move lots of
its information into Appendix D. Create a xref to Appendix D.
