Continue integrating fixes from Sunday's meeting.

* /trunk/doc/book/book/ch08.xml: Further refinements.

Some fixes for Chapter 6 (made at 36000 ft).

* ch08.xml: fix two typos that were preventing compilation.

* ch00.xml, ch01.xml: various fixups from review session feedback.

* ch02.xml: new tweaks, and a new section on the limitations of
mixed-rev working copies.

Fix typo.

* /trunk/doc/book/book/ch06.xml: Fix typo.

Changes from the meeting on Sunday. Stopped at svn export.

* /trunk/doc/book/book/ch08.xml: Incorporate edits from review.

Fix "svn commit --force" example to actually specify "--force". We
have Julian Foad <julianfoad@...> to thank for this.

Spelling fix (issue 1123), noticed by the sharp eyes of Andriy Palamarchuk.

Spell "System" correctly.

The great markup sweep. This sorta kinda seems to be what the tools
guys at ORA told me, but I think that they didn't understand what I
really wanted to do. In any case, this does parse, but I believe I'm
going to have to do more monkeying around with this at a later date.
But for our weekend review, it should be just fine. There are still a
number of TODOs, and a couple of sections left to markup.
