Some ID renaming work.

* tools/chunk-stylesheet.xsl: Turn on "use.id.as.filename".
* tools/idsub.py: New, a little script to crudely substitute ids.
* en/ID-REMAP: Catch up with ID changes following the merge of "1.2-work",
plus fix up now I realize that '+' is not a valid id character.

Delete sussman's temporary branch, it has been merged.

Merge Ben's temporary book branch into trunk.

This is the result of running

* src/en/book/appc.xml: total rewrite of autoversioning appendix for 1.2.

New (temporary) file: A mapping of ids from old numeric to new symbolic forms.
Now that I've finished constructing the map data, I'll get started on a script
to apply it. Meanwhile, this is here for review.

* en/ID-REMAP: Add.

* en/book/ch03.xml: Add missing <command> around title.

* en/TODO: Another.

* en/book/ch02.xml: Replace a plaintext link with an <xref/>.

Update "1.1" to "1.2" in <subtitle>For Subversion 1.x</subtitle>

* en/book/book.xml: Update.
