Rework book-dist.py to be a little more flexible (and fix a tarball generation
bug).

Remove a bunch of uninteresting copies of 'en' files. (If relevant

Stylesheet tweak. Again. One day I'll be happy, I swear.

Fix httpd.conf programlistings -- all programlisting and screen sections
should be left-aligned in the first column.

Remove stale outline.txt file.

* src/en/book/ch09.xml: Add --version switch for svnserve.

Factor out some duplication into a common file.

* tools/base-html-stylesheet.xsl:
New file, containing bulk of old content from...

Remove the empty colophon.
