Version Control With Subversion Source

Migrate the bulk of the locale-independent Makefile out of the locale-specific
"en" directory, and into "tools".

* tools/Makefile.base: Move en/Makefile here, add comment.
(L10N_REVISION): New make variable, to localize word generated by Makefile.

* en/Makefile: Replace with a simple include of Makefile.base.

Major attack of refactoring on Makefile. Amongst other things, generate the FO
file only once when building both PS and PDF, switch the XML validator to
xmllint, since it is faster and less complex to invoke (and so it should now
work regardless of platform), remove several obfuscating variables, tidy up
overlength lines, and move some more things into the "You may want to
customize these paths" section.

Book Spanish. Corrected closing filename tag.

Add an XSL transform which produces and outline of the id attributes throughout
the book. Intended as a developer tool.

* tools/make-id-outline.xsl: New file.

Comparing standards throughout the book, in general <simplesect> elements do
not have id attributes, so remove them from the two that do.

Book Spanish. Domingo Suárez Torres provided four translated
paragraphs for the second chapter.
