What I know about it:

Normally first time connection with postgrey results in a greylisting 
message and time out.

But lately there are mailservers that still try every second to drop the 
same email (although everyone knows what greylisting is about).
I reported this behavior to the author and he considered this as 
annoying behavior too, resulting in putting these kind of mailservers on 
the internal black list.
