#260 BAN reason

Basically, the concept is to make it possible to do the
following command:
/msg chanserv reason #channel

and get the following output:
You were banned by: <nickname|*handle>
Because: [reason]

I think it'd be pretty simple to code, and useful to
the general user as well.
