The big 1.2.0 bug was caused by some misplaced HTML
tags... running the generated HTML through a validator
would catch these sorts of things, and is generally
good practice.

Here are some patches that clean up the generated HTML
and should make the pages validate, except for a few
things that didn't seem too critical to me (e.g., use
of MS-specific extensions, such as <td nobr> and <table
cols=n>, missing ALT attributes in <img>, stuff of that
nature).

I think the only structural problem remaining is the
handling of hidden options (SMOPT_TYPE_HIDDEN)... the
<input type=hidden> tag for those ends up inside a
<table>, but not within a <td>. Hopefully, that won't
cause any rendering problems... it's hidden, after all :)
