I got it to work - this is what I had to do.

Copy the functions directory one directory below the index.php.

Then in imap.php I had to take out the '.' In the require_once lines.
I am wondering if it might be my version of php  - PHP Version 4.0.1pl2

I have yet to get options to work.

I too am having problems upgrading from 1.0.6 to the latest 1.2.2.  I'm
getting the same fatal error that others were getting telling me that:

Fatal error: Failed opening required '../functions/url_parser.php'
(include_path=') in ./functions/strings.php on line 212

I have the latest TAR (as of today, 1/2), so I'm assuming there aren't any
permissions issues (as referenced in previous e-mails about this issue).
When I changed the "include" string on line 212 of strings.php to read
/full/path/to/functions/strings.php it works fine, but eventually crapps out
at another relative path reference to  ../functions/.  So, am I to assume
that it's an issue with using ../ and ./ instead of full path names?  If so,
then why does 1.0.6 work so well when it references includes the same way?
