This is expected, and is correct. All folders ARE subfolders of INBOX to
courier.

No. that isn't expected. I can fix that, but it's not a problem with the
default settings as much as it's a problem with how we present
folders/subfolders for creation..

I still recommend that people press D. I think it's the smarter thing.
