It seems to me it is a user permission problem to create the output/ subfolder.
I tried on a MacBook Pro with Python 2.6 as a normal user within his
/Users directory and it worked.
Check the permissions in your /Users/pentest/Desktop/sqlmap-0.6.4/
folder, it needs to be rwx for the user 'pentest'.
