Summary:     The ``.unique`` flag on :class:`.Index` could be produced as ``None``
if it was generated from a :class:`.Column` that didn't specify ``unique``
(where it defaults to ``None``).  The flag will now always be ``True`` or
``False``. [ticket:2825]
