Notes(Help)

Hi,

 I'm writing a notes module that will work for "credit note" and "debit 
note". Now I'm working on the addnote function(add), It works well, It 
generates the "Add Credit Note" and "Add Debit Note" pages, but It doesn't 
shows the section where you add the products. Please help me to find why 
it's happening?
 I think the problem is at the "update_form" function in the invocation to 
display_form. 

 I attached my module files.
