[SQL-Ledger-users] customer form date field?

Hello,:
How do I include a date field at customer form?
An error msg concerning that it (the one I added) was formatted as int4 =

instead of date appears....
The others added , text and int, are ok.
Please, a cook recipe, as I know zero about perl. (newbie)
