[SL] Server error when adding new item (add part)

Yesterday I downloaded and installed sql-ledger (2.6.22) on a Mandriva
server 2006.

The install proceeded ok and seems to be running fine.

I was able to edit the preferences, edit the system defaults, add to
the Chart of Accounts and add customers.  But when I try to add a
part, I get a server error after I enter the details and click on
save.

           The server encountered an internal error and was unable to
complete your request. Either the server is overloaded or there was an
error in a CGI script.

 If you think this is a server error, please contact the webmaster    Error 500

I've used it (2.6.3) on a Mandrake 8.2 and had no problems.  Also
tried to install it on a Mandriva 2006 and its working without the
problem.

 Can't see anything in the apache logs.  Any idea on what I might be
missing and where I might try to find possible causes?
