#1 Additional approval on submitted bans

Provide the option of requiring more than one admin to approve a ban before it is added to the main ban list.

Could involve making a seperate table for submitted bans with a required fielding needing two or three approval ticks before moving to main ban list table.

Would be handy for sites wanting to support multiple server providers looking for reassurance bans are properly reviewed.
