The default proxy handling does not conform to the way recommended by RFC 3261.

The default proxy URI should be optionally added to the initial route set and Route header in the requests.

In my opinion, the default proxy (optionally) should give way to the top Record-Route header as the destination for requests in a dialog, if that is given during the dialog establishment.

There is a problem of loose routing: should the proxy URI be added to the initial route set with "lr" parameter even if it doesn't feature it explicitly? Otherwise we get strict routing behavior and I presume, that's not desirable. Maybe another boolean option tag should be added to control that.
