Here is my guess (after seeing it happen many time here):
Do you have a virus scanner running? It is often from a virus
scanner removing a file before Fluffy has passed it off. Fluffy
handles virus scanning by packing the mail into a zip file,
calling the command-line scanner, and waiting for the
response code. If the virus scanner detects and deletes the
file before Fluffy has finished with it, Fluffy throws a fit. It
expects to find a file that doesn't exist anymore!
That's my guess, anyway. I've only seen it happen when
under heavy virus load, and the virus scanner doesn't behave
properly (happens with all of them somtimes). I'm using
Sophos right now, and it can go days without a restart, and
then 8 times in a few hours.
I have a network services monitor that checks Fluffy every 60
seconds. If Fluffy doesn't respond, the monitor issues
a "Restart" to the service, so I don't have to worry about it
(WhatsUp Gold is fantastic!).

so it isn't there any more. And fluffy gets upset.

I get this error quite a bit. At least once a day. I am running
AVG 7.0 Network Edition. I was under the impression that
this Anti-virus scanner for Fluffy is not implemented yet. I
guess it is, and screws up in its logic.
