They are normally set by the device manufacturer. Note that early
versions of the SMART standard (SFF, and ATA-3 and perhaps ATA-4)
supported an ATA_SMART_WRITE_THRESHOLDS (Feature register= 0xd7) command
that could be used to set thresholds. This was specifically intended for
the case where the factory-floor-set thresholds turned out to be wrong,
and needed to be reset in the field. Probably most disk vendors still
support the command, since it's useful in manufacturing.
Currently smartctl does not support this command. It wouldn't be hard to
add, but it would probably get little use.
There are at least some cases where vendors distribute modified firmware
