#97 Index handler patch

This patch replaces the modified <a
apache handler</a> that several people, and sites, have
used over the years.

Basically, if you set two vars, you create an alternative
splash page that all anon users would be directed to when
they hit the / index page.

Steps to be able to use this:

1. Apply the patch.
2. add the following to the sql upgrades file, and the slash
sql data import:

INSERT INTO vars (name, value, description) VALUES
('index_handler_noanon','home','The shtml page to call if a
user is anon and index_noanon is set');
INSERT INTO vars (name, value, description) VALUES
('index_noanon','0','Redirect all anonymous users to
index_handler_noanon instead of index.shtml. Set to 1 to
activate, 0 to remove.');

This is a very simple mod that I know quite a few people
have asked about in the past.
