It's simply a human oversight. It happens. Someone changes a plugin or internal code, then the schema needed for it, but then forgets to append the change to the updates file. Typically, the best thing to do, when you find this, is file a bug report in the slashcode SF homepage. If you attach a diff -N -U3 -xCVS -r to the bug report so that all they have to do is download an apply the patch (ie it's easier for them) then it'll be pushed into the src tree that much faster.
