#3290 Second page is screwed up in Nested mode

I've noticed for a long time that the second page is
messed up when displaying comments in Nested mode.
Today, for the 'Too Big to Test' thread, some comments
that should have been on page (2) didn't display at all.

This bug has been around for a long time (at least a
year, I think).

It's not just the second page, a recent article had almost 2000 comments
on it, and the first 17 pages in 'nested' mode were exactly the same!
Nested mode is FUBAR and has been for years.
