README file needs to direct the user to
1) create a database for use with the app ( default name siwt )
2) create a user with select and insert access to the database created in #1 ( default user/passwd -> siwt/siwt )

In the menu.php file I had to add " .php " to the links to get them to work. I think this is a local Apache/PHP setting and may not have to be done on all systems as part of the install.
