I got portsentry to compile under Mac OS X 10.3.2 with the SUPPORT_STEALTH flag. Not really complicated: everything is there but features.h, which appears not to be needed.
It launches, and reports binding of the ports as under linux. But I haven't seen any attackalerts.
I am running with OS X's builtin firewall activated: this appears to be nothing more than libwrap compiled into xinitd.
Has anyone else tried? Are there specific reasons why stealth mode is not supposed/expected to function under Mac OS X?
