The Semantic Forms "edit with form" tab links to this URL: 

http://mydomain.com/wiki/index.php?title=User:XYZ&action=formedit (There is a form on every User page.)

Edit with Form tab is most definitely a Semantic Forms feature, it's supposed to appear on every page that contains a form.

 The tab disappears for me when Group Permissions Manager is installed and enabled. I suspect that the Group Permissions Manager is blocking actions that aren't specifically allowed. So before I take my case up with Ontoprise I seek to learn more about how the "Edit with Form" function works from this group...does it depend upon other actions, for example?

Just to clarify, the "formedit" action comes from the Group Permissions Manager extension, not from Semantic Forms; so if no one else responds to this thread, you should probably ask the same question on one of the Ontoprise forums.
