#1 Make RoHS declaration statement in a consistent format

In version 1.1 of IPC 1752-1, the RoHS declaration statements are drop down items in the PDF form. They are listed as below:
1 - Item(s) does not contain RoHS restricted substances per the definition above
2 - Item(s) contains RoHS restricted substances above the limits per the definition above and is not under exemption
3 - Item(s) does not contain RoHS restricted substances per the definition above except for lead in solders and selected exemptions, if any
4 - Item(s) does not contain RoHS restricted substances per the definition above except for selected exemptions
5 - Item(s) is obsolete, no information is available
6 - Item(s) is unknown, no information is available

Because there is a leading number in the statement, it would be easier for a program to parse which statement is answered. In Scriba, the statements does not have those leading numbers. We would like to have a consistent format and add the leading number in the statements; the same format that being used in previous version of IPC 1752-1 form.

In addition, there should be a validation so that a user cannot create an XML with all six RoHS statements answered "true". Currently, there is no such validation.
