- changed the "d_" prefix for member variables to
the more common "m_"

- moved all action, menu, selection and otherwise GUI
related code from Table to TableView
(remark: some functionality needs to be split
into selection related and non-related parts
and moved back to Table later)
- implemented proper handling of globals in Table/TableView
- some other minor changes to faciliate GUI backend
separation
