SciDAVis - Improvements SVN-temp

- changed the "d_" prefix for member variables to
the more common "m_"

- moved all action, menu, selection and otherwise GUI
related code from Table to TableView
(remark: some functionality needs to be split
into selection related and non-related parts
and moved back to Table later)
- implemented proper handling of globals in Table/TableView
- some other minor changes to faciliate GUI backend
separation

- fixed/improved column cell validity handling

- column cells are now invalid by default if they
get created by ColumnPrivate::resizeTo
- TableModel now returns a red '-' for invalid cells
instead of "invalid"

- merged changes to NSIS script, README and INSTALL.html from 0.2.x branch
- increased version to 0.1.4 (updated splash etc.)

- changed NSIS script to the version I used for 0.2.0-beta1
- fixed bug 2093825: installer always sets a desktop icon
