I've installed graphviz, and verified that it is on my path, by running dot -V. That returns the following output: dot - graphviz version 2.28.0 (20110507.0327)
But I am still getting the following error messages in the errorlogs:

Warning: Failed to run dot.
Download dot version 2.2.1 or versions greater than 2.4
from www.graphviz.org and make sure that dot is either in your path
or point to where you installed Graphviz with the -gv option.
Generated pages will not contain a diagramtic view of table relationships.
Using schemaspy version 5.0.0.

i'm running into the same issue. Here is my error output

java.lang.ClassNotFoundException: com.mysql.jdbc.Driver
Failed to load driver 'com.mysql.jdbc.Driver'
This entry doesn't point to a valid file/directory: [/mysql/mysql-connector-java-5.1.6-bin.jar]
Use the -dp option to specify the location of the database
drivers for your database (usually in a .jar or .zip/.Z).
