When the instruction

<xsl:attribute name="a" namespace="b">c</xsl:attribute>

is used, and the namespace declaration

xmlns="b"

is in force (that is, b is the default namespace),
then the newly-created attribute is placed in the null
namespace rather than in the namespace b.

Present in 6.5, 7.0, and all earlier releases.

Source code fixed (both branches).

Test case added: nspc33
