Updating ipr disclosures to what is suggested in RFC7991.

Adding in first xml3 conversion of the saratoga tsvwg
draft, converted via https://author-tools.ietf.org/.

Since it's a few years on from the bulk of the work, and had
to be updated from xml v2 to v3, which reformatted everything,
this lives here in svn as this separate file.

Also updated Chris Jackson's contact details.

Chas spotted a line missing from the REQUEST ascii diagram.
The bits are defined a couple of pages earlier.

Updating this as a stopgap while I work on getting things
into the new xml format.

Upping version number for future release. Still valid in current xml checker!
