Converting from V2 to V4 can involve more than just changing the version number in the header. First the Perceptual intent needs to have the black point remapped so the L* is about 3. The text tags need to be converted to Multi-Localized Unicode. And the proper required tags need to be put in place. The ICC specifications are the place you need to look.
The IccProfLib library is an excellent tool for loading in tags and accessing data. You will need to write an application yourself to do the actual work though.
