I suggest to try the search pattern "\<_(?:(?:_(.*))|([A-Z]+))" on source files. Some places will be found where names begin with two underscores or an underscore and an uppercase letter.

Examples:
- _LOGGING_H_
http://rxvt.svn.sourceforge.net/viewvc/rxvt/trunk/src/logging.h?revision=1768&view=markup

- _RXVT_H_
http://rxvt.svn.sourceforge.net/viewvc/rxvt/trunk/src/rxvt.h?revision=1758&view=markup

This does not fit to the expected naming conventions of the C language standard.
