#9 Provide an allocator parameter to thread_copy

When a thread is manipulated by value, a 'new' must be done on the fly, to store the thread copy in thread_copy. It is unfortunate that there is no allocator parameter in thread_tree, for this purpose. One might even use 'swap' with the temporary thread passed as a value on the stack.
