I am using Ubuntu Hardy with the kernel 2.6.24-23-rt and jackd 0.116.1 and I'm not having xruns or midi timing problems.

With my laptop internal sound card (Intel) I can get a 8ms latency without xruns. With an external USB card I launch JACK with the following options:

/usr/bin/jackd -v -R -P70 -dalsa -r44100 -p256 -n3 -D -Chw:1,0 -Phw:1,0 -m -i2 -o2 -Xseq

I've tried the kernel 2.6.27-3-rt, but it doesn't work with dual core machines: http://ubuntustudio.org/8-10_release_note

The only thing I want to emphasize: Don't use the 2.6.27 rt-kernel, it's
midi timing is absolutely broken and unusable.
