bump to 4.99.30

Better UTF-8 support for XML output:
- XML Docbook does not assumes input to be ISO-8859-1 anymore but respects the --charset option
- --documenttitle used in XML Docbook output too

Test

- The charset now defaults to ISO-8859-1 (not UTF-8)
- Characters above 160 won't be escaped anymore which broke UTF-8
- Latex uses the 'inputenc' package if the option --charset was specified
