#76 Section numbering does not work consistently

In some output modes you get always section numbers for
headers (Latex singledoc) and in some you get never
(html multidoc).

The section numbering is also linked with the option
--sectionnameonly that gives the user less flexibility.
It should be better to control the section numbering
with a separate --option.
