#17 Fix or remove the insert sample link in hover

The link doesn't work because its handler can't find the document nor the region (offset) where to insert the sample.

Solution are either:

• Find a way to give those informations (they are given by the spell engine when handling spelling)
• Remove the "Insert Sample" links from the docs
